From spoken visit to finalized chart note

Step 1: Capture

The visit starts. Scribemarrow listens.

The physician opens a session at the start of the encounter. Ambient audio capture begins immediately with no additional steps. There is no activation phrase and no device to position in the room.

Step 2: Process

Clinical language, properly structured

Scribemarrow's clinical model extracts entities from the conversation: chief complaint, history, examination findings, assessment, plan. Each maps to the correct SOAP section using clinical vocabulary, not general language.

  • Correct ICD-10 code suggestions included
  • Drug names, dosages, and anatomical terms preserved
  • Audio not retained after processing completes

Step 3: Deliver

Note in your EHR. Physician signs.

The completed note is written directly into the open EHR encounter via SMART on FHIR integration. The physician reviews the structured output, makes any edits, and signs. No copy-paste, no separate inbox to manage.

  • Direct write to Epic, Athenahealth, eClinicalWorks, and others
  • Physician signs and finalizes the note as with any other chart entry
